Schedule / Job Type

Contract | Days | 5x8 Hours (8:00–16:30)

Key Responsibilities

Provide physical therapy services to home health patients in accordance with physician-approved care plans
Complete and maintain OASIS assessments as required for home health documentation
Chart and document patient progress accurately and in a timely manner
Float to other locations as needed based on operational needs
Collaborate with interdisciplinary team members to ensure continuity of patient care

Required Qualifications

Active Physical Therapist license; Virginia state license required to be in hand at time of submittal or proof of compact license
Home Health experience required
OASIS experience required
Specialty experience within the last year required
SSN required at time of submittal
One supervisory reference required at time of submittal
Local candidates are welcome and accepted at the same rate

Preferred Experience / Skills

Experience with EPIC or similar electronic health record systems
Strong communication and interdisciplinary collaboration skills
Ability to manage a flexible schedule, including floating assignments as needed
