Every life you touch here makes our entire community stronger.A Primary Care Provider (PCP) provides comprehensive, long-term care to an adult patient population, ages 18 and older. The provider diagnoses and treats a variety of diseases and injuries in a primary care setting and provides appropriate medical evaluation, diagnosis, and treatment. The PCP also provides medical services to patients within an assigned service area and coordinates discharge planning with the Case Manager for admitted patients.
